The Utah Chapter of the Society for Public Health Education (USOPHE) is committed to maintaining the privacy of the personal information of our members. This document outlines what that means and how any personal information we collect may be used.
When an individual selects to become a member of USOPHE, certain personal information is collected including name, email address, and phone number. This information is voluntarily given and is used to help the USOPHE board contact members relating to their account memberships, send timely and useful information through newsletters and other communications, and upcoming event information. Information such as email and phone number is also included in the membership directory, viewable only by paid members for purposes of networking. Personal information may also be collected from both members and non-members of USOPHE if an individual is registering for an event and/or paying for items from the online store for the purposes of communicating about and fulfilling their orders. Any information collected in this way is not shared with anyone except the individuals on the USOPHE Board responsible for fulfilling their orders and is not added to our list of contacts unless the individual selects to become a USOPHE member.
If an individual is signing up for a group membership, they will be entering personal information on behalf of the individuals that will be part of the group membership. Their information will be included on the membership directory. The person administrating the group membership (also known as bundle coordinator) is responsible for collecting consent from the individuals that they are providing personal information for before they sign up for a group membership. The same applies for an individual who is registering multiple people for an event. USOPHE does not share this information with anyone other than the individuals on the USOPHE Board who are responsible for fulfilling their orders.
USOPHE does not use any third-party applications to collect or share personal information.
